Witajcie!


Szukam informacji na temat starania się o dziecko przez mężczyznę leczonego biologicznie (Imfliximab). Gastrolog enigmatycznie stwierdziła, że w przypadku kobiet wymagane jest zaświadczenie że nie będzie się starać o dziecko w trakcie leczenia i do pół roku po jego zakończeniu, natomiast mężczyzn to nie dotyczy. Pytanie brzmi czy leczenie biologiczne mężczyzny może mieć jakiś negatywny wpływ na rozwój dziecka? Czy bezpieczniej odłożyć starania o dziecko na pół roku po zakończeniu terapii?

Wydaje mi się, że dotyczy to i kobiet i mężczyzn. Byłam leczona tym przez 5 lat i wydaje mi się, że na ulotce tak było napisane. postaram się ją odnaleźć i potwierdzić

Notabene ciekawa jestem czy któraś z leczących się biologicznie zaszła w ciąże, w tym okresie 6 miesięcznej karencji po ostatniej dawce? Czytałam na zagranicznym forum, że dziewczyny podczas leczenia decydują się na ciąże i rodzą zdrowe dzieci. Ja zamierzam odczekać te 6 miesięcy. Ale tu znów pojawiają się dylematy kolejne - 6 miesięcy i co już można?, czy asekuracyjnie wydłużyć ten okrres np. do 8 echhhh

Jak rozumiem nie ma stosownych badań stwierdzających, czy preparat jest bezpieczny w ciąży i na wszelki zalecane jest odłożenie starań na czas leczenia. Ale to dotyczy tylko kobiet.

Male fertility problems not observed during infliximab treatment

Key point: A small retrospective case series of four male patients with ankylosing spondylitis treated with infliximab (Remicade—Centocor) noted that these patients had no fertility problems during infliximab treatment.

Finer points: Sophia Paschou, MD, and colleagues from the University of Ioannina, Greece, reviewed medical records of male patients with ankylosing spondylitis to determine which patients were treated with infliximab and had fathered children while receiving treatment. The investigators identified four male patients with a mean age 33.5 years (range 30–39 years) and mean disease duration of 14.8 years (range 9–19 years) who had fathered a total of six children while receiving infliximab infusions. Patients received infliximab 5 mg/kg every 8 weeks; one patient had been given 10 infusions when he fathered the first child and the other three male patients had been given 14 infusions at the time of conception of the first child. Two patients also fathered additional children, with one fathering the second child after receiving 24 infliximab infusions and the other fathering the second child after receiving 31 infliximab infusions. The investigators reported that all six infants were healthy, all of the men were in clinical remission when they fathered children, and none of the men required fertility assistance. Serum hormone levels (luteinizing and follicle-stimulating hormones and testosterone) were not available, so the effect of infliximab on these levels was unknown. The investigators concluded that this small case series should provide male patients treated with infliximab some reassurance that fertility will not be affected.

What you need to know: Limited data are available on the effects of infliximab on fertility in men. In vitro data by Suominen and colleagues have suggested that tumor necrosis factor-alpha (TNF-alpha) promotes cell survival during spermatogenesis and that this effect may be blocked by infliximab, a TNF-alpha antagonist. Other investigators have reported negative effects of infliximab on male fertility. La Montagna and colleagues reported that two of three male patients with ankylosing spondylitis treated with infliximab had reduced sperm motility. Both patients had previously fathered children. Mahadevan and colleagues also reported on the effects of infliximab in 10 male patients with inflammatory bowel disease. They noted that infliximab therapy decreased sperm motility and altered sperm morphology, with therapy reducing the number of normal oval forms. In addition to a limited amount of information, the data on the effects of infliximab on male fertility appear to be conflicting. Larger, prospective, well-designed studies are needed to truly determine the effects of infliximab on male fertility.

What your patients need to know: Inform male patients that limited data are available regarding the effects of infliximab therapy on fertility. Educate them on the available data and tell them that the risks and benefits of infliximab therapy must be considered when determining if therapy is appropriate in male patients planning on having children.
